Riverwood achieves Health Care house certification
In an effort to get better the Price & quality of care delivery, Riverwood Healthcare Center has pursued & achieved Health Care house (HCH) certification out of the Minnesota section of Health. A patient-centered health care house is a philosophy of care which supports health & wellness When placing control of health care choices in the hands of the patient. Each patient selects a preferred primary care suppliers & the care team works together to treat the whole patient, connecting patients by resources within a health care Administration & the society. "by the implementation of Patient Centered Health Care house in our Riverwood dispensaries , we've developed a team approach to coordinating care for patients by chronic or complex health conditions," said Cindi Baker, Riverwood chief operations officer.
