Apology ordered after toddler given wrong vaccine at medical centre according to : TVNZ

Two registered nurses at a bustling medical centre combined to "depart from accepted standards of nursing care" and give a 15-month-old boy the wrong vaccine. A second registered nurse was asked to check the vaccines, but failed to spot the error. A registered nurse selected the vaccines from the medical centre's refrigerator prior to the child's visit, but mistakenly selected Infanrix-IPV instead of the Act-HIB vaccine. Infanrix-IPV was given to the boy and the first registered nurse noticed the mistake when checking the immunisation record and the boy's Well Child Book. While one of the nurses said the medical centre was busy that day, Ms Duggal said by following correct vaccine protocol and re-checking the vaccine selected, the mistake would not have been made.

The nurse asked another registered nurse to check the vaccines, but neither picked up on the mistake. Two registered nurses were found to have breached a 15-month-old boy's rights after he was accidentally given the wrong vaccine. AdvertisementInfranrix-IPV is listed on the New Zealand National Immunisation Schedule as a booster given at the age of four years, after a child has received all three doses of Infanrix-hexa. The vaccine he was given, Infranrix-IPV, was not harmful, the Immunisation Advisory Centre said.

Ms Duggal said one nurse, RN B, mistakenly chose the latter vaccine, Infanrix-IPV, to be given instead of the Act-HIB. Ms Duggal said RN C had a responsibility to ascertain the age of the child and check that RN B had selected the right medicines. Two busy nurses have been found to have breached patient rights by giving the wrong vaccine to a 15-month-old boy last year. "If he'd been in another country with a slightly different [immunisation] schedule to New Zealand, he could well have received that vaccine or something very similar anyway. She said RN B had not told her the boy's age.
